Project description
The main objective of the project is to ensure comprehensive and integrated support for patients in protected discharge. Home care services, temporary professional protective assistance and specific training for home service professionals will be provided.
Collaboration with hospital and local health authorities will be promoted in order to define and test protocols for integrated care shared by health and social services for patients. To achieve this, accredited private entities and Third Sector entities will be involved in the provision of services.
The project, named “A CASA CON CURA” (“AT HOME WITH CARE”), is divided into five specific interventions involving 625 people.
