Actions for independent living and de-institutionalisation for the elderly

Project description

The project included a series of infrastructural investments aimed at preventing the institutionalisation of the elderly by providing them with innovative housing and equipment. The aim was to enable the elderly to maintain an autonomous and independent life, with access to home services, which ensured continuity of care. The socio-medical care model provided simplified pathways for access to services and multidimensional and integrated care.
One of the main strategies was the strengthening of territorial domiciliary services, with the creation of networks covering groups of flats not integrated in a structure. This allowed the elderly to remain in their own area, receiving the services required to meet their needs.Alternatively, the Service Local Areas (Ambiti Territoriali di Servizio, ATS) proposed the gradual redevelopment of public residential facilities into groups of independent flats.
The use of home automation, telemedicine and remote monitoring elements helped to improve the effectiveness of the intervention, enabling constant supervision and timely assistance. Activities involved 260 elderly beneficiaries through the following projects: “CO.abiti_AMO”, “Nuova Roma 3”, “Ristrutturiamo Casa Giada”, “Casa Vittoria”, “Stiamo insieme anche se..”.
